All participants and guardians (if applicable) are required to sign this waiver BEFORE the first class

By participating in class the participant/ guardian understands that there is a risk of personal injury in the course of instruction and, with this knowledge, agrees to assume the risk of any injury and damages to participants during training.

Specifically, the participant agrees to hold

BJJ Battalion harmless and all other individuals, organizations, sponsors, promoters, operators, hosts, instructors, associations, schools, owners, officials, directors, employees, and other participants connected with the event from all losses, damages, injuries, causes of actions, claims, or complaints in the event that the participant is damaged or injured in any way during the participation, instruction and/or performance of any exercise or during any activity associated with the event location or during transit to or from the event.

Participant/ and or guardian further agrees to strictly obey instructors and observe safety rules.

Because of the physical demands of this instruction, the participant understands that he/she must be in good physical condition to participate in the event. Participant understands that in case of injury, the only medical treatment BJJ Battalion will provide is first aid.

Participant agrees that any pictures, audio, or visual recordings taken of him/her in connection with BJJ Battalion can be used for publication, promotion, articles, shows, and advertisement without additional consent and without compensation at this time or any other time.

By participating I hereby release and agree to hold BJJ Battalion harmless from and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses, and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the club, or that may otherwise arise in any way in connection with any services received from BJJ Battalion.

I understand that this release discharges BJJ Battalion from any liability or claim that I, my heirs, or any personal representatives may have against the club with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from BJJ Battalion.

This liability waiver and release extends to the club together with all owners, partners, and employees. I have read and understood this release and agreement and agree to its provisions. I am not under the influence of any drugs, alcohol, or other intoxicants. I am not suffering from any illness or incapacity. I am over 18 years of age. (If not over 18 years of age, parent or guardian must sign.
